Personal Information Consent Form
In order for us to provide you an effective service, we need to keep a record of your personal information. To comply with the Data Protection Act (1998) and the General Data Protection Regulations, we have to explain to you how we use your data and ask for your permission.
The information we record and use
This include your name, age, address, email and phone number. Others include your sex, marital status and church membership. All other questions are voluntary, although answering them will be of benefit to you.
How we store your information
All personal information is treated as private and confidential by all staff and
volunteers. It is recorded on a database and/or in a paper file.
You have the right to see any information that we hold about you, and to have your details removed.
How we may share your personal information
We can only share your personal information to a third party if it will benefit you and you give us the permission to do so.
